Faculty/Administrator Regalia Request and Participation Form

All Full-time or part-time/adjunct Faculty and Administrators participating in one of the following ceremonies must submit the Regalia/Participation Form.

All Request and Participation Forms requesting rental regalia must be submitted by Sunday, March 16th so as not to incur a late fee. Forms will only be accepted online. If you own your own regalia and the University is not renting your regalia, you can continue to submit the form until Friday, May 2nd.

Late Orders Only: If you are a full-time or part-time faculty or administrator ordering rental academic regalia after the deadline beginning March 17 through May 2, please enclose a $45.00 late fee to cover the rental and "special handling" charges incurred from the rental company. Late fee must be paid to the Office of Commencement Services prior to the special order. Please make your check payable to the University of La Verne and mail it to the main campus address, Attn: Commencement Services, SRC 101. Thank you!
