Thank you for the decision to join us in celebrating your accomplishments! Please fill out the following form completely.

To participate in one of the main campus 2008 Spring Commencement Ceremonies, you must fill out this form completely and submit it online. DEADLINE TO SUBMIT THE STUDENT PARTICIPATION FORM IS FRIDAY, MAY 9TH.
Returning this form by the final deadline is mandatory. If you do not indicate your decision to participate in advance by using this form, you will not be permitted to participate in one of the main campus Spring Commencement Ceremonies and will not be able to pick-up your guest admittance tickets in May. Guest tickets will only be available to those that have filled out this form. No exceptions!
If you are uncertain which ceremony you are assigned to participate in, check out the Commencement Ceremony/Degree Chart for verification prior to filling out this form. You have been assigned to a specific ceremony.
If you do not submit this form by the MAY 9TH deadline, you must appeal through a Letter of Explanation as to why you are late ($40.00 appeal fee will apply) beginning Monday, May 12 through Friday, May 16 to participate in a ceremony. Regalia cost plus additional late regalia fees will apply to your order. No appeals will be accepted after May 18th.
Students that have been cleared to graduate in Spring, look for your name in the official Commencement lists posted on the Spring Commencement web site in late April. No correspondence will be sent by mail. All commencement information will be posted on the ULV Spring Commencement web site and updated regularly.

Important:To guarantee that your name will be listed in the official Commencement Program, your Student Participation Form must be completed and submitted on-line by Friday, May 9, 2008. Letter of Explanation appeal and late fees will apply beginning Monday, May 12th.

Candidates who fail to complete the on-line form by the deadline date of May 9th deadline, must:

Submit a Letter of Explanation appeal to the Office of Commencement Services, SRC Room 101, as to why your participation should be granted after the deadline was missed, along with a $40.00 late fee. This fee is non-refundable. Additional costs for regalia must be paid if appeal is granted.

Note: Candidates accepted after the deadline (May 9th) are not guaranteed to be listed in the official Commencement Program.

No Letter of Explanation appeal will be accepted regarding Student Participation in Spring 2008 ceremonies after Friday, May 16th.
