Hours for Fall and Spring Semesters and Fall, Winter and Spring Terms:

Closed in the summer for walk-ins. If you are studying in the summer, call to leave a message at (909) 593-3511 ext. 4254. The nurse will return your call within 24 hours.

Health Insurance

The University of La Verne provides all international students with basic and supplemental health insurance coverage, the cost of which is paid for at the beginning of each semester/term. This insurance covers basic medical costs. Students should consider acquiring a primary insurance policy. All students on an F-1 or J-1 visa, are required to take insurance coverage provided by the university. Innoculations

The University of La Verne does not have requirements regarding immunizations or a physical examination students must have before registering for classes. Most American students who begin university studies will have the following immunizations:

Accessing Health Care

The University of La Verne Student Health Center provides medical personnel and facilities to promote health care. Each full-time international student may use these facilities during the semester/term in which s/he is registered (The Health Center is closed during the summer term). The center provides a staff of one full-time doctor, a physician's assistant, graduate nurses, and a part-time nurse practitioner. It is open to provide services to ULV students during the Undergraduate academic year from September until June. If you are an international student at ULV, you should have health coverage. International students are required to pay for insurance when they register for classes.

We recommend that you check with the health ministry or department of your government and ask what medical professionals recommend to you for an extended stay in the United States.

Questions Regarding Health and Medical Coverage: Summer, Vacations, and Optional Practical Training.

Please read the following carefully. All international students are required to purchase university insurance while enrolled at ULV. "Medical Service" is the Insurance coverage provided through the university. The "Health Center" on the central campus provides medical information and services. This coverage is included in the tuition and fees paid by undergraduate students. Graduate students pay separately for this coverage. The questions are divided into two sections:

A) Semester-based and B) Term-based.

A. Term Based: Questions about Medical services and the Student Health Center for students in a program on the term system (e.g. MBA, Gerontology, etc programs):

1. Will my ULV medical services cover me after I graduate?

It will only cover you until the end of the "period of coverage." Using the 2005-2006 academic year that coverage was:

Fall term: September 3-December 18
Winter term: December 18-March 19
Spring term: March 19-June 4
Summer term: June 4-September 5

If you graduate December 18, your coverage ends that day. If you graduate March 19, your coverage ends that day. If you graduate May 28, your coverage ends June 4; if you graduate August 19, your coverage ends September 5.

If you are interested in coverage after you graduate, you will need to purchase an medical services plan. Remember that you will not have access to the Student Health Center after you graduate. 2. Will the Student Health Center be open during the summer?

No. The Student Health Center is closed for office visits from the last day of Spring classes until the first day of class in the semester-based programs, usually the first day after Labor Day, in early September.

3. If I get sick in the summer, what should I do?

If you need medical assistance during the summer you should:

a) Call the Student Health Center (Extension 4254) and leave a message and a nurse will call you back. You may consult with the nurse over the telephone.

Or you may

b) Call and leave a message and go immediately to the Central Avenue Urgent Care Center ( http://www.pvhmc.org/montclair/body_central.asp )

Be sure to take your university ID with you. If you have it, also take the medical services card that the medical services company mailed to you.

If you are given a prescription, take it to Star Drugs, 3280 North Garey Avenue, Pomona. It is located on the corner of Garey and Foothill. Again, take your student ID and health medical services card (if you have it).

4. In the term programs we can take a vacation during any of the terms, after we have been enrolled for three consecutive terms. Do I have coverage during my vacation period, whenever it is?

No. You do not have coverage during your vacation period. If you are interested in coverage during your vacation, you will need to purchase an medical services plan. Remember that you will not have access to the Student Health Center during your vacation. 5. If I am taking classes part time, do I have coverage?

Yes, if you have paid the medical services fee. Remember that you must have paid the medical services and student health center fee to have coverage. Check your receipt to be sure you have been charged.

6. When must I pay the medical services and health center fees?

You will pay at registration. It is very important that you pay this fee by the end of the first week of class, to be sure that you have the coverage.

7. I am starting classes at ULV during the four week 2nd summer session that begins in late August. Do I have medical services and health center coverage?

You will have coverage if you pay the medical services and health center fees when you register. The summer term coverage ends in late August so you will need to pay the medical services and health center fees for both the Summer and Fall terms. We recommend you check to see that you pay for coverage for both the short Summer 2 and the longer Fall term.

B. Semester-based: Questions about Medical Services and the Student Health Center for students in a program on the semester system (e.g. all undergraduate programs, some graduate programs):

1. Will my ULV medical services cover me after I graduate?

Fall term: August 26, 2005-January 31, 2006
Spring term: January 31, 2006-August 26, 2006

If you graduate December 18, your coverage ends January 31. If you graduate May 28, your coverage ends August 26.
