Courier Delivery Form Instructions


Purpose:
To assist the Mail Center in fulfilling your expedited courier needs, and to alleviate any confusion with the address or delivery information.

  1. Date: Enter the date the Courier Delivery Form was prepared
  2. Origination Information (Grey Box) This is information about you and the sending department
    1. Contact Name: your name
    2. Dept: your department
    3. Chargeable Acct #: list the 8 digit number(s) to which the goods or services should be charged. (i.e. 11111111)
    4. Telephone Ext: your extension
    5. E-Mail: your email account
  3. To: what is the name of the company/agency that the package is going to
  4. Attn: What is the individual or department that you are directing the parcel to?
  5. Street 1: Address of the Recipient
  6. Street 2: Address of the Recipient, if more than one is needed
  7. City: city of the Recipient
  8. Suite: The Suite # (where applicable)
  9. State/Province: put the state abbreviation if in the U.S. or the Province out of the U.S.
  10. Zip or Zip+4: the zip code
  11. Country: if out of the Country
  12. Telephone # of Recipient: The phone # where the recipient can be reached.
  13. Signature Release: The ability to track the package is not hindered. No signature will be needed to deliver the parcel; it allows the courier to leave your package at a safe designated location without having to obtain a signature from the recipient if they are not available to sign for the package. Please initial to verify acknowledgement, if yes.
  14. Value of contents: include this amount if you would like insurance (at an added cost) for an amount over $100.00.
  15. Delivery Need Level: please choose the service type you require.
  16. Preferred Courier: please choose your preferred courier... if left open, the Mail Center will make an informed decision.
