Procedure:

Requestor completes the ULV Record Storage Label in permanent marker for each box and places it inside a sheet protector and attaches it to the front of the carton:

Requestor fills out Records Pick-Up Receipt.

| 3.0 | Requestor emails the purchasing department to arrange pick-up. | ||||||||||||
| 4.0 | Purchasing signs both copies of the Records Pick-up Receipt then leaves one with the requestor. | ||||||||||||
| 5.0 | Purchasing places cartons in storage. | ||||||||||||
| 6.0 | Purchasing enters placement of cartons into inventory. |
