The University needs your cooperation in order to keep ULV a safe place. It is very important to report accidents and injuries immediately to Campus Safety so first aid or emergency help can be provided and so they can promptly investigate the facts and document the circumstances. It is important to note that ULV does not provide accident or medical insurance coverage for visitors.
Accident/Incident Reporting Procedures
A student or visitor who has had an accident, needs emergency medical attention or requires other assistance, should call the Campus Safety emergency number at ext. #6666. For non-emergencies, call ext. 4950. The responding officer will assess the situation, coordinate assistance and write a report.
Student Activities
Students participating in voluntary extra-curricular activities and some events are required to sign a waiver/disclaimer and consent for treatment form. If the student is younger than 18 years, a parent or guardian must sign the waiver/disclaimer in order for the student to participate. Waivers are available on this Website.
Statements
University employees should not make any statement regarding the University's responsibility or liability for any injury or loss. Consult with the Risk Manager or Campus Safety.
