Complete the following information:

- Club/Organization Roster with a minimum of six full-time undergraduate students (Download) .xlt
- Club/Organization Constitution/Bylaws
- Calendar of Events

If you fail to submit this information, by the deadline you may be subject to loss of privileges such as but not limited to:
- Use of ULV facilities
- Funding from ASULV
- Access to your on-campus account
- Ability to advertise your club or organization events on campus

Submit this information to the Office of Student Life (located in the 2nd floor of the ULV bookstore) by Friday, September 19th, 2008 by noon to be considered for funding from ASULV. Otherwise, you can turn paperwork in anytime to be considered recognized by the Office of Student Life.
